1. Introduction

These Terms of Service ("Terms") govern your use of the Tansy Concierge Appointment Service Pilot Program ("Pilot Program") provided by Tansy AI, Inc, d/b/a Tansy ("Company"). By participating in the Pilot Program, you agree to these Terms.

2. Service Description

The Pilot Program offers personalized scheduling services for medical, dental, and vision appointments. Your participation is subject to these Terms and the consent form you have signed.

3. User Responsibilities

As a participant, you agree to:

• Provide accurate, current information, including contact details and health preferences.

• Pay the monthly fee of $3.50 for the first three months, increasing to $17.50/month after the Pilot Program.

• Use the service responsibly and notify us of changes in your communication preferences.

4. Service Limitations

We facilitate appointment scheduling but do not guarantee the quality of medical services from healthcare providers. Our role is limited to coordination, and we are not responsible for the medical care you receive.

5. Fees and Payments

You agree to be billed monthly and authorize us to charge your payment method. If your payment fails, service may be suspended until resolved.

6. Privacy and Data Protection

We take the protection of your personal and health information seriously. Your participation is subject to our Privacy Policy and the consent you've provided regarding the use of your health information for scheduling purposes.

7. Termination

You may opt out of the Pilot Program anytime by written notice. Tansy reserves the right to terminate participation if you violate these Terms or the consent agreement.

8. Feedback and Improvements

By participating in the Pilot Program, you agree to provide feedback when requested. Your feedback and de-identified data may be used to improve the service.

9. Limitation of Liability

Our liability is limited to the fees paid for the Pilot Program, except where such limitation is prohibited by law. We are not responsible for indirect or consequential damages arising from your participation.

10. Changes to the Terms

We may update these Terms periodically. Any changes will be communicated to you, and continued participation constitutes acceptance of the revised Terms.

These Terms are governed by the laws of the United States.
